ACP community workshops
Palliative Care WA runs free, informative and interactive community workshops on advance care planning and palliative care in Perth and regional WA thanks to funding from the WA Department of Health.
We all want to have a say in what happens in the last part of our lives. By making plans that cover your future care, lifestyle, health and finances, you’re not only working out what you want, you are also making things easier on those around you. This process is called Advance Care Planning.
What is covered?
Everyone who attends one of our workshops will receive a comprehensive resource pack that contains all the information and handouts you need to participate in the workshop and some advance care planning resources. The workshop will cover:
- what’s involved in advance care planning
- exploring what matters to you and what your priorities are for the last stage of life
- how to have a conversation about your wishes with your family, loved ones and health care providers
- what is involved in palliative care and when it should be offered
- how to complete documents such as an Advance Health Directive and Enduring Power of Guardianship.
Online workshops
We also offer our advance care planning workshop online, via Zoom. Everyone who attends an online workshop receives our resource pack by post after the workshop. You’ll also receive a link to download the resources, if you’d like to do this.
Get in touch with us if you’d like to join online but are new to Zoom workshops. Your local library, neighbourhood centre or community resource centre may also be able to help you join an online session.
My Future Care ACP workshop resources
ACP resources are provided to people who attend an online Advance Care Planning community workshop run by PCWA. These resources are also available to anyone interested in having a say in their future care.
Please see HERE.
Safe Space workshops
From time to time we provide a Safe Space Workshop online. That means they are only open to people with a chronic condition or terminal illness, as well as those who care for family or friends with a chronic condition or terminal illness. While the core information on advance care planning is the same as other workshops, we recognise that some people will feel more comfortable sharing questions and concerns with those in similar circumstances… within a ‘safe space’. We respectfully ask anyone who does not fit into this group, to not register for one of our Safe Space Workshops.
How do I find a workshop?
To find out if one of our face-to-face or online workshops is scheduled near you, go to our Events page. Workshops are being added all the time, so check back regularly.
Alternatively talk to us about partnering with us to host one for your community or organisation. It can be exclusively for your audience, or we can promote the workshop more widely and open it up to others in the general community. We come to you, and we can provide a customised workshop to suit specialised audiences.
To find out more about hosting a workshop download the information flyer here.
