Esperance community members learnt all about Advance Care Planning last week when PCWA team members Sabena and Julia visited the south-east coastal community.
Our two workshops – My Future Care Advance Care Planning workshop and an Advance Health Directive document completion workshop – and individual ACP support sessions over three days were very well received.
