National Advance Care Planning Week 2022
We all want to have a say in what happens in the last part of our lives. By making plans that cover our care, lifestyle, health and finances, we work out what we want, as well as making things easier on those around us. This process is called Advance Care Planning.
Palliative Care WA provides two free advance care planning services to assist the WA community:
- Advance Care Planning Community workshops – These interactive and information workshops cover what’s involved in advance care planning, exploring what matters most to you, how to have conversations about your wishes with your family and loved ones, and information on completing advance care planning documentation.
- Advance Care Planning Support Service – For those who have attended a workshop but need a bit of extra assistance to take the next steps, this new service provides personalised and practical help to progress or complete their advance care planning in their home, at our office, or at another suitable venue. Having a discussion online is also an option.
