Expressions of interest open for new WA Advance Care Planning Community of Practice
Palliative Care WA is inviting expressions of interest to join the new WA Advance Care Planning (ACP) Community of Practice (CoP), commencing in November 2026.
The initiative will bring together practitioners involved in ACP education and support to share knowledge, experiences and resources, and strengthen ACP practice across the state.
The Community of Practice is funded through WA Primary Health Alliance (WAPHA) under the Greater Choices for At Home Palliative Care Program.
What will the Community of Practice involve?
The CoP will provide a forum for practitioners to connect with others working in ACP, share practical experiences, discuss challenges and opportunities, and explore ways to improve ACP education, resources and activities.
Meetings will be held monthly for approximately 60 minutes, with members encouraged to actively participate and contribute their knowledge and experiences.
The CoP will also provide opportunities to build professional connections and strengthen collaboration between practitioners working across different settings and locations in Western Australia.
Who can join?
Expressions of interest are welcome from practitioners who are actively involved in delivering or facilitating advance care planning education or support in Western Australia.
Members will receive further information about the CoP’s Terms of Reference, meeting dates, format and participation requirements.
How to express your interest
If you’re involved in ACP education or support and would like to connect with others working in this space, we encourage you to express your interest.
Download the Expression of Interest form here Please email the Expression of Interest form to: Julia.Kennington@palliativecarewa.asn.au.
We look forward to bringing together a diverse group of practitioners to share knowledge, strengthen professional connections and support the ongoing development of ACP practice across Western Australia.
