ACP Support Service reintroduced – four options available to WA community members
The Advance Care Planning Support Service (ACPSS) has been reintroduced as a trial thanks to funding from the WA Department of Health.
The Palliative Care WA service is currently running for six months in the first half of 2025 to support WA community members to progress their Advance Care Planning (ACP) documentation, in particular their Advance Health Directive (AHD).
Advance Care Planning is the process of making plans that cover an individual’s future care, lifestyle, health and finances so they have a say in what happens in the last part of their life.
The ACPSS can help individuals to:
- identify what they want for their future care
- complete ACP documentation
- work out how to talk to loved ones about future care wishes.
The new pilot service has four options for support for individuals who have attended an ACP education workshop through PCWA (or other organisations), have considered their end-of-life wishes, values and preferences and are ready to complete their documents.
The service is free and provides four options of assistance:
- AHD document support workshops (open to all community members)
- Office location 1–1 support (open to all community members)
- In-home 1–1 support (referral required)
- Online or phone 1–1 support (open to all community members but especially those in rural or remote locations)
The first workshop in option 1 above will be held on Monday 24 February 2025 from 10.00am to 12.30pm at the Cancer Council WA Seminar Room, Upstairs, Milroy Lodge, 15 Bedbrook Place, Shenton Park.
