Advance Care Planning Support Service
Page is under review and construction
Advance Care Planning is the process of making plans that cover your future care, lifestyle, health and finances so you have a say in what happens in the last part of your life. It’s important to start your Advance Care Planning early in life when you can speak and decide for yourself. If your personal situation, health, or lifestyle changes so too can your choices for your future care. Which means your Advance Care Planning can be an ongoing process that changes to suit you.

The Advance Care Planning Support Service, funded by the Department of Health as a pilot project in 2025, supports individuals to progress their Advance Care Planning. The service helps people have a say in what happens in the last part of their lives. Details on how to book for the support service will be available soon.
Taking the next steps
If you have previously attended one of our Advance Care Planning Community Workshops but haven’t yet been able to take the next steps, then our support service can help. This personalised and practical service can help you:
- identify what you want for your future care
- complete your advance care planning documentation
- work out how to talk to your loved ones about your future care wishes.
We can come to your home, meet you in our office or talk with you somewhere else that is suitable. If you are not located in the Perth metropolitan area or it is difficult for you to meet us in person, we can provide an online appointment. The Advance Care Planning Support Service is free and fully confidential.
Before using this service we encourage you to complete one of our free Advance Care Planning Community Workshops. They provide an introduction to all aspects involved in advance care planning. You can find out if one is scheduled near you on our events page.
Please note: We don’t provide any legal, financial or health advice and are not able to arrange witnesses to documentation.
Call us on 1300 551 704 to find out more or make a booking.
A previous pilot of the service was funded by Carers WA. Please see below for reports from that program.
