Advance Care Planning is the process of making plans that cover your future care, lifestyle, health and finances so you have a say in what happens in the last part of your life. It’s important to start your Advance Care Planning early in life when you can speak and decide for yourself. If your personal situation, health, or lifestyle changes so too can your choices for your future care. Which means your Advance Care Planning can be an ongoing process that changes to suit you.

The Advance Care Planning Support Service is a joint pilot project between Palliative Care WA and Carers WA that has been designed to support individuals to progress their Advance Care Planning. The personalised mobile service helps people have a say in what happens in the last part of their lives and uses a friendly conversational approach.
Our Advance Care Planning Support Service has been an outstanding success in its first year of operation, with a 94 per cent document completion rate! Taking the next steps
If you have previously attended one of our Advance Care Planning Community Workshops but haven’t yet been able to take the next steps, then our support service can help. This personalised and practical service can help you:
- identify what you want for your future care
- complete your advance care planning documentation
- work out how to talk to your loved ones about your future care wishes.
We can come to your home, meet you in our office or talk with you somewhere else that is suitable. If you are not located in the Perth metropolitan area or it is difficult for you to meet us in person, we can provide an online appointment. The Advance Care Planning Support Service is free and fully confidential.
Before using this service we encourage you to complete one of our free Advance Care Planning Community Workshops. They provide an introduction to all aspects involved in advance care planning. You can find out if one is scheduled near you on our events page.
