Advance Care Planning Support Service
Advance Care Planning is the process of making plans that cover your future care, lifestyle, health and finances so you have a say in what happens in the last part of your life. It’s important to start your Advance Care Planning early in life when you can speak and decide for yourself. If your personal situation, health, or lifestyle changes so too can your choices for your future care, which means your Advance Care Planning can be an ongoing process that changes to suit you.

The Advance Care Planning Support Service, funded by the Department of Health as a pilot project in 2025, supports individuals to progress their Advance Care Planning. The service helps people have a say in what happens in the last part of their lives.
If you have previously attended one of our Advance Care Planning Community Workshops but haven’t yet been able to take the next steps, then our support service can help you to:
- identify what you want for your future care
- complete your advance care planning documentation
- work out how to talk to your loved ones about your future care wishes.
The Advance Care Planning Support Service comprises the following:
- Advance Health Directive (AHD) document support workshops (open to all community members) See our events section for dates.
- Office location 1–1 support (open to all community members)
- In-home 1–1 support (referral required)
- Online or phone 1–1 Support (open to all community members but especially those in rural or remote locations)

If you work for an organisation and would like to refer a client to the In-home 1–1 support, please use this referral form.
