Our Advance Care Planning Support Service Report July 2022 to June 2023 showing the success the service has had in its first full year of operation, has recently been published.
Thanks to Carers WA’s very generous funding and the excellent work our support staff have undertaken over the year, the service has gone from strength to strength, with the number of people helped continuing to increase.
The service has received very positive feedback from participants, such as:
“If it wasn’t for this session, I would never have known about this service. Very grateful for the possibility to have a home visit, especially to give my elderly mother more comfort and feel in a safe place to be able to express her wishes.”
“The at-home help gently guided me to accurately complete the Advance Health Directive and I now feel confident about my advanced care.”
The service is currently being extended to provide a more intensive focus of assistance specifically for carers.
