My Future Care Online – an introduction to Advance Care Planning for carers

Would you like some personalised and practical help with your and your loved one’s Advance Care Planning?
Advance Care Planning is the process of making plans that cover future care, lifestyle, health and finances so you and your loved one have a say in what happens in the last part of life. It’s important to start Advance Care Planning early in life when you and your loved one can speak and decide for yourselves.
If a person’s situation, health or lifestyle changes so too can their choices for future care. This means your Advance Care Planning can be an ongoing process that changes to suit your circumstances.
Taking the next steps
Our online workshop introduces our personalised service that can help you and the person you are caring for to:
- Identify want is wanted for future care.
- Complete advance care planning documentation.
- Work out how to talk to important people in your lives about your future care wishes.
Thanks to funding from Carers WA, this workshop is free for anyone either in a caring role or a care recipient and living in Western Australia. There is a fee for participants from elsewhere in Australia. We are not able to offer this workshop to international participants.
Who is this for?
This online workshop is suitable for any individual in a caring role, whether it be in a formal or in-formal capacity. It is also suitable for individuals who are care recipients regardless of age, health or background. You don’t need any prior knowledge of Advance Care Planning to take part.
How does it work?
This free, confidential and personalised service can be provided in your home, at our office, at another suitable venue, or online.
