My Future Care – Advance Care Planning Community Workshop – open to the public

October 9 @ 1:30 pm - 4:00 pm

We all want to have a say in what happens in the last part of our lives. By making plans that cover our care, lifestyle, health and finances, we work out what we want, as well as making things easier on those around us. This process is called Advance Care Planning.

This workshop at Darling Range Hub organised by Matthew Hughes MLA, will cover information on what is involved in Advance Care Planning, how to get started, and how to talk with your loved ones about your end-of-life wishes. It is a practical and interactive workshop where you will have the chance to get your questions answered.

Who is this for?

This face-to-face workshop is suitable for any community members regardless of age, health or background. You don’t need any prior knowledge of Advance Care Planning to take part. To book your place please contact Karen Beale at Matthew Hughes MLA’s office via karen.beale@mp.wa.gov.au or via 9293 4747.

Want to organise a workshop?

We provide free community workshops on Advance Care Planning and Palliative Care thanks to funding from the WA Health Department. If you would like to organise a workshop for your own community group, organisation or workplace please contact us on info@palliativecarewa.asn.au.
