AHD document completion workshop in Geraldton – open to the public

You are invited to attend a FREE AHD document completion workshop at the Queen Elizabeth II Seniors and Community Centre Geraldton that will assist you to complete your Advance Health Directive. It is ideal for community members to take part in one of our My Future Care workshop first, but this isn’t a requirement.

During this workshop, hosted by the City of Greater Geraldton, we will work through each page of the AHD allowing you to work towards the completion of the document to a stage where it is ready for signing, witnessing and presenting to your health professional.

Participants should have considered their end-of-life wishes, values and preferences and should be ready to complete their document in preparation for attending this workshop.
