AHD document completion workshop – at the South Metropolitan Health Service

You are invited to attend a FREE AHD document completion workshop at South Metropolitan Health Service Older Adult Community Integrated Care Hub in Success that will assist you to complete your Advance Health Directive. It is ideal for community members to take part in one of our My Future Care workshop first, but this isn’t a requirement.

During this workshop we will work through each page of the AHD allowing you to work towards the completion of the document to a stage where it is ready for signing, witnessing and presenting to your health professional.

Participants should have considered their end-of-life wishes, values and preferences and should be ready to complete their document in preparation for attending this workshop.
