Advance Health Directive document completion workshop in Shenton Park

You are invited to attend a FREE Advance Health Directive (AHD) document support workshop that will assist you to complete your AHD.

It’s ideal for community members to take part in the My Future Care Advance Care Planning workshop first, but this isn’t a requirement.

During this workshop we will guide you through each page of the AHD allowing you to work towards the completion of the document to a stage where it is ready for signing, witnessing and presenting to your health professional.

Participants should have considered their end-of-life wishes, values and preferences and should be ready to complete their document in preparation for attending this workshop.
