Reimagining Advance Care Planning

A recent journal article recommends a more social inclusive approach to Advance Care Planning that involves clinicians and health care workers focusing on what matters to people instead of concentrating on harm reduction and avoiding unwanted treatments.

The authors advocate an approach to care that takes into account people and where they are best cared for, their happiness and wellbeing, and the happiness and wellbeing of their primary carer.

Among their recommendations they suggest that ACP conversations focus on what matters most in life to each person while they are well and when they become less well, and how they can gain support from their social network to achieve these priorities.
