Statewide service based within paediatric public hospital providing consultative service for children with non-malignant conditions aged 0-16 years. The core team consists of medical, nursing and social work staff with administrative support. Multidisciplinary care based on the unique needs of the patients, carer and family when facing a life-limiting prognosis including:  Symptom management  Addressing emotional and social concerns  Assistance with clinical decision making and advanced care planned  Assist primary medical teams with care coordination  Aid access to equipment and resources  Providing quality care for those approaching the end of life  Aim to support choice of place of care at end of life  Transition to adult health services  Bereavement support Care is provided through shared care between primary care, specialist and community services.
