Statewide service based within paediatric public hospital providing consultative service for children with non-malignant conditions aged 0-16 years. The core team consists of medical, nursing and social work staff with administrative support. Multidisciplinary care based on the unique needs of the patients, carer and family when facing a life-limiting prognosis including: Symptom management Addressing emotional and social concerns Assistance with clinical decision making and advanced care planned Assist primary medical teams with care coordination Aid access to equipment and resources Providing quality care for those approaching the end of life Aim to support choice of place of care at end of life Transition to adult health services Bereavement support Care is provided through shared care between primary care, specialist and community services.
