This forum aims to help you plan ahead so your family and health care providers know what you want at end of life. Information covered will include the elements of advance care planning and how to have the conversations with your loved ones on your end of life wishes. You will also have opportunity to get your questions answered by our experts.
Understanding Advance Care Planning will be presented by Lana Glogowski. As our CEO Lana has been leading our drive to increase awareness and understanding of advance care planning and palliative care in our community. Lana has specialist expertise in all aspects of advance care planning and presents our advance care planning workshops to community groups across WA.

This forum is FREE, however registrations are essential.

There are three Understanding Advance Care Planning online forums scheduled. While the material covered will be similar, each one is designed for different audiences. The other two are:
- Tuesday 2 June, 10–11:30am: Suitable for anyone in the general community
- Thursday 4 June, 10–11:30am: Suitable for families, friends and carers of people with chronic disease or terminal illness
