My Future Care – Advance Care Planning Community Workshop – in-person event
Tuesday 27 September
1.00pm to 3.00pm
My Future Care – Advance Care Planning in-person community workshop
Seminar Room, Cancer Council WA, 15 Bedbrook Place, Shenton Park
We all want to have a say in what happens in the last part of our lives. By making plans that cover our care, lifestyle, health and finances, we work out what we want, as well as making things easier on those around us. This process is called Advance Care Planning.
This face-to-face workshop will cover information on what is involved in Advance Care Planning, the new Advance Health Directive forms, how to get started, and how to talk with your loved ones about your end-of-life wishes. It is a practical and interactive workshop where you will have the chance to get your questions answered.
Who is this for?
This workshop is suitable for any community members regardless of age, health or background. You don’t need any prior knowledge of Advance Care Planning to take part.
Thanks to funding from the WA Department of Health, this workshop is free for anyone living in Western Australia. There is a fee for community members from elsewhere in Australia. We are not able to offer this workshop to international participants.
You must register for this face-to-face workshop here on Eventbrite. After you register we will send you a resource pack that you need to participate in the workshop. If you register by Tuesday 20 September we will post you the resource pack. Alternatively, you can obtain a pack on the day.
You are welcome to contact us at Palliative Care WA if you have any questions on 1300 551 704 or firstname.lastname@example.org.
