SUMMARY:My Future Care - Advance Care Planning Online Community Workshop - Safe Space
DESCRIPTION:We all want to have a say in what happens in the last part of our lives. By making plans that cover our care\, lifestyle\, health and finances\, we work out what we want\, as well as making things easier on those around us. This process is called Advance Care Planning. \nThis online workshop will cover information on what is involved in Advance Care Planning\, how to get started\, and how to talk with your loved ones about your end-of-life wishes. It is a practical and interactive workshop where you will have the chance to get your questions answered. \nWho is this for? \nThis is one of our ‘safe space’ workshops. It is only open to people with a chronic condition or terminal illness\, as well as those who care for family or friends with a chronic condition or terminal illness. \nWhile the core information on Advance Care Planning that is covered is the same as other workshops that we run\, we recognise that some people will feel more comfortable sharing questions and concerns with those in similar circumstances … within a ‘safe space’. \nWe respectfully ask anyone who does not fit into this group\, to not register for this workshop. \nThanks to funding from the WA Department of Health\, our workshops are free for anyone living in Western Australia. There is a fee for community members from elsewhere in Australia. SUMMARY:Death Cafe open to LGBTQI+ people and allies
DESCRIPTION:Deathcafe.com is a worldwide movement started in London in 2010 by Jon Underwood and Sue Barsky Reid\, based on the ideas of Swiss Sociologist Bernard Crettaz. \nThe objective of the deathcafe.com movement is “to increase awareness of death with a view to helping people make the most of their (finite) lives”.  \nOf course\, cake\, tea or coffee always helps the conversation flow! \nPalliative Care WA recognises the importance of death cafes and their role in helping people increase their awareness of death and dying. \nLGBTQI+ people and allies are invited to this death cafes\, with the theme of ‘Muffins\, Mugs & Memories’.

DESCRIPTION:If you have completed one of our Advance Care Planning workshops\, you are invited to attend a FREE ACP document completion workshop organised by the City of Rockingham that will assist you to complete your Advance Health Directive. \nDuring this workshop we will work through each page of the AHD allowing you to work towards the completion of the document to a stage where it is ready for signing\, witnessing and presenting to your health professional. \nParticipants should have considered their end-of-life wishes\, values and preferences and should be ready to complete their document in preparation for attending this workshop. \nLimited places available in each workshop.
