It’s important to start Advance Care Planning early in life when you and your loved one can speak and decide for yourselves. \nIf a person’s situation\, health or lifestyle changes so too can their choices for future care. This means your Advance Care Planning can be an ongoing process that changes to suit your circumstances. \nTaking the next steps \nOur online workshop introduces our personalised service that can help you and the person you are caring for to: \n\nIdentify want is wanted for future care.\nComplete advance care planning documentation.\nWork out how to talk to important people in your lives about your future care wishes.\n\nThanks to funding from Carers WA\, this workshop is free for anyone either in a caring role or a care recipient and living in Western Australia. There is a fee for participants from elsewhere in Australia. We are not able to offer this workshop to international participants. \nWho is this for? \nThis online workshop is suitable for any individual in a caring role\, whether it be in a formal or in-formal capacity. It is also suitable for individuals who are care recipients regardless of age\, health or background. This process is called Advance Care Planning. \nThis online workshop will cover information on what is involved in Advance Care Planning\, how to get started\, and how to talk with your loved ones about your end-of-life wishes. It is a practical and interactive workshop where you will have the chance to get your questions answered. \nWho is this for? \nThis is one of our ‘safe space’ workshops. It is only open to people with a chronic condition or terminal illness\, as well as those who care for family or friends with a chronic condition or terminal illness. \nWhile the core information on Advance Care Planning that is covered is the same as other workshops that we run\, we recognise that some people will feel more comfortable sharing questions and concerns with those in similar circumstances … within a ‘safe space’. \nWe respectfully ask anyone who does not fit into this group\, to not register for this workshop. \nThanks to funding from the WA Department of Health\, our workshops are free for anyone living in Western Australia. There is a fee for community members from elsewhere in Australia. DESCRIPTION:We all want to have a say in what happens in the last part of our lives. By making plans that cover our care\, lifestyle\, health and finances\, we work out what we want\, as well as making things easier on those around us. This process is called Advance Care Planning. \nThis workshop at the Connect Victoria Park\, 5 Mackie Street\, Victoria Park\, will cover information on what is involved in Advance Care Planning\, how to get started\, and how to talk with your loved ones about your end-of-life wishes. It is a practical and interactive workshop where you will have the chance to get your questions answered. \nWho is this for? \nThis face-to-face workshop is suitable for any community members regardless of age\, health or background. You don’t need any prior knowledge of Advance Care Planning to take part.
