It is ideal for community members to take part in one of our My Future Care workshop first\, but this isn’t a requirement. \nDuring this workshop we will work through each page of the AHD allowing you to work towards the completion of the document to a stage where it is ready for signing\, witnessing and presenting to your health professional. \nParticipants should have considered their end-of-life wishes\, values and preferences and should be ready to complete their document in preparation for attending this workshop. \nLimited places available in each workshop.
