FOLLOW UP ADVANCE CARE PLANNING WORKSHOP IN PERTH

Come along to a free community workshop about completing advance care planning documentation.

This workshop is for people who have attended a Palliative Care WA Advance Care Planning and are considering making an Advance Health Directive, and appointing an Enduring Guardian and/or Attorney.  Please note this workshop will provide general guidance only, not personal professional advice.

WHEN:    Friday 28 June,  10am—12pm

WHERE:   Seminar Room, Cancer Council WA, 15 Bedbrook Place, Shenton Park WA. Enter via upstairs carpark. Parking is free.
