DESCRIPTION:Join us at our Cup Half Full – Grief Café during National Palliative Care Week on 14 May 2026 at Herdsman Lake Discovery Centre. \nThe café will run from 10.00am to 12.00pm and welcomes people to share about loss and grief over coffee and cake in a safe space to talk and listen. \nThe café will be facilitated by Sabena Lund\, who has training and experience in holding group spaces and conversations. \nTea\, coffee and morning tea provided. \nAll welcome to attend. SUMMARY:My Future Care – Advance Care Planning Online Workshop
DESCRIPTION:We all want to have a say in what happens in the last part of our lives. By making plans that cover our care\, lifestyle\, health and finances\, we work out what we want\, as well as making things easier on those around us. This process is called Advance Care Planning. \nThis online workshop will cover information on what is involved in Advance Care Planning\, how to get started\, and how to talk with your loved ones about your end-of-life wishes. It is a practical and interactive workshop where you will have the chance to get your questions answered. \nWho is this for? \nThis online workshop is suitable for any community members regardless of age\, health or background. You don’t need any prior knowledge of Advance Care Planning to take part. \nThanks to funding from the WA Department of Health\, our workshops are free for anyone living in Western Australia. There is a fee for community members from elsewhere in Australia. DESCRIPTION:You are invited to attend a FREE AHD document completion workshop at South Metropolitan Health Service Older Adult Community Integrated Care Hub in Success that will assist you to complete your Advance Health Directive. It is ideal for community members to take part in one of our My Future Care workshop first\, but this isn’t a requirement. \nDuring this workshop we will work through each page of the AHD allowing you to work towards the completion of the document to a stage where it is ready for signing\, witnessing and presenting to your health professional. \nParticipants should have considered their end-of-life wishes\, values and preferences and should be ready to complete their document in preparation for attending this workshop. \nLimited places available in each workshop.
