SUMMARY:My Future Care – Advance Care Planning Community Workshop – open to the public
DESCRIPTION:We all want to have a say in what happens in the last part of our lives. By making plans that cover our care\, lifestyle\, health and finances\, we work out what we want\, as well as making things easier on those around us. This process is called Advance Care Planning. \nThis workshop hosted by the Westcoast Community Centre at the Grove Library\, will cover information on what is involved in Advance Care Planning\, how to get started\, and how to talk with your loved ones about your end-of-life wishes. It is a practical and interactive workshop where you will have the chance to get your questions answered. \nWho is this for? \nThis face-to-face workshop is suitable for any community members regardless of age\, health or background. You don’t need any prior knowledge of Advance Care Planning to take part. DESCRIPTION:You are invited to attend a FREE Advance Health Directive (AHD) document support workshop that will assist you to complete your AHD. \nIt’s ideal for community members to take part in the My Future Care workshop first\, but this isn’t a requirement. \nDuring this workshop we will work through each page of the AHD allowing you to work towards the completion of the document to a stage where it is ready for signing\, witnessing and presenting to your health professional. \nParticipants should have considered their end-of-life wishes\, values and preferences and should be ready to complete their document in preparation for attending this workshop. \nLimited places available in each workshop.
