DESCRIPTION:Advance Care Planning is the process of thinking about and sharing your wishes regarding care\, health and ﬁnances\, in case you’re not able to make and communicate your decisions in the future. \nThis online workshop\, during Dying to Know Week\, will cover information on what is involved in Advance Care Planning\, how to get started\, and how to talk with your loved ones about your end-of-life wishes. It is a practical and interactive workshop where you will have the chance to get your questions answered. \nWho is this for? \nThe workshop is suitable for any community member regardless of age\, health or background. You don’t need any prior knowledge of Advance Care Planning to take part. \nThanks to funding from the WA Department of Health\, our workshops are free for anyone living in Western Australia. There is a fee for community members from elsewhere in Australia. DESCRIPTION:You are invited to attend a FREE Advance Health Directive (AHD) document support workshop that will assist you to complete your AHD. \nIt’s ideal for community members to take part in the My Future Care workshop first\, but this isn’t a requirement. \nDuring this workshop we will guide you through each page of the AHD allowing you to work towards the completion of the document to a stage where it is ready for signing\, witnessing and presenting to your health professional. \nParticipants should have considered their end-of-life wishes\, values and preferences and should be ready to complete their document in preparation for attending this workshop. \nLimited places available in each workshop.
