DESCRIPTION:Advance Care Planning is the process of thinking about and sharing your wishes regarding care\, health and finances\, in case you’re not able to make and communicate your decisions in the future. \nThis workshop\, hosted by the SMHS Older Adult Health Hub\, will cover information on what is involved in Advance Care Planning\, how to get started\, and how to talk with your loved ones about your end-of-life wishes. It is a practical and interactive workshop where you will have the chance to get your questions answered. \nWho is this for? \nThis face-to-face workshop is suitable for any community members regardless of age\, health or background. You don’t need any prior knowledge of Advance Care Planning to take part. To book your place please visit HERE. \nWant to organise a workshop? \nWe provide free community workshops on Advance Care Planning and Palliative Care thanks to funding from the WA Health Department.
