SUMMARY:AHD document completion workshop – at Cockburn Seniors Centre
DESCRIPTION:You are invited to attend a FREE AHD document completion workshop at the Cockburn Seniors Centre that will assist you to complete your Advance Health Directive. It is ideal for community members to take part in one of our My Future Care workshops first\, but this isn’t a requirement. \nDuring this workshop we will work through each page of the AHD allowing you to work towards the completion of the document to a stage where it is ready for signing\, witnessing and presenting to your health professional. \nParticipants should have considered their end-of-life wishes\, values and preferences and should be ready to complete their document in preparation for attending this workshop. \nLimited places are available in each workshop.
