The End of Life Care Program within the WA Department of Health is aiming to empower Western Australians to make choices about their future health and personal care that reflects their values, beliefs and preferences with these new resources. The WA Minister for Health, the Hon Amber-Jade Sanderson, will officially launch the revised Advance Health Directive.
